I/we acknowledge, understand, and appreciate that participation or observation of the above  mentioned activities constitutes risk to me/us of serious injury, including permanent paralysis, and death.  I/we shall assume the risk inherent in these activities and shall not hold the academy, its employees, officials, and representatives liable for any death, injury, loss, damage, cost or expenses arising from participation or observation.  I/we have permission to participate in the MasterClass Rugby Academy.  I/we give the aforementioned MasterClass Rugby Academy permission to render medical attention should it be deemed necessary.

In signing this application the athlete or parent certifies that the applicant and/or participant is in good health physically and mentally.

Safety equipment is to be used by participants.

I/we acknowledge that I/we have read and  understood the terms and conditions of this application and agree to abide by the terms and  conditions.
